Admission is open to referrals from Opportunities for Ohioans with Disabilities (OOD) and the local boards of developmental disabilities in Portage and Summit Counties.
Once your referral and authorization for services are received, you will be matched to a career navigator based on your preferences, location, service requested and caseload availability.
Pathfinder does not work with individuals that require behavior interventions. If it is learned at any point in the referral or services that you are on a formal Behavior Support Plan, a copy of your plan will be requested and reviewed to determine your level of needs and whether you can be served.
If you are self-referred or referred by a source other than OOD or a county board, you will be provided with contact information to refer yourself for services.
If there is a waiting list, we will tell you right away of any potential delays and the service timeframe. Services will be provided on a first-come, first-served basis, with the opportunity to change providers given as an option. If a job opportunity becomes available that is a match for you, you may receive priority services during this time.
